Forms are created in the Form Designer and can be added from Templates, from the Designer Folder, or from a Flow. When creating a Form, select a Form template, provide a name and description, and then use the Form Designer to add and configure controls. Form controls can be added from the Toolbox, named, resized, and arranged in the workspace. --- Follow the next Steps to create a Form in the Decisions Platform. 1. Start creating a Form by doing one of the following: - Use a Form template available in the **Normal Form** section. See the table below for more information about each template. | Form Type | Description | | --- | --- | | Form | Creates a blank Form | | Approval Form [template] | A Form containing two Buttons: Approve and Deny | | Mobile Form [template] | A Form that creates a user interface designed for interaction from mobile devices | | Mobile Form (iPhone) [template] | A Form that creates a user interface designed for interaction from iPhone devices | | Section Form [template] | A Form containing a HeaderContainer control that may be expanded once clicked on | | 4 Column Form [template] | A Form containing 4 Columns upon creation | - In a Project, go into any Designer Folder and click CREATE FORM from the **Global Action Bar** to go directly to the Form Designer. - Use the **Show Form** step in the [Flow Designer](https://documentation.decisions.com/version-10/docs/create-flows) and click PICK or CREATE FORM on the **Properties** panel. ![](https://cdn.document360.io/6ef8bcc1-6489-4486-9ad1-83acff7e5df0/Images/Documentation/image-1782933681388.png) 2. From the **Favorites** section, name and describe the Form and select Create. For example, the **Normal Form** template displays as follows: ![](https://cdn.document360.io/6ef8bcc1-6489-4486-9ad1-83acff7e5df0/Images/Documentation/Normal%20Form%20Template%20-%2010.1.png) 3. In the Form Designer, the **Toolbox** panel contains all the [Form Controls](/version-10/docs/form-control-glossary) that can be added to the Form workspace. Click and drag a Form control and place it on the Form. 4. When adding a control to the Form workspace, enter a name for the control and press Enter to set the name. 5. Form Controls can be resized using the resize handles on the edges or corners. Resizing into objects is not allowed and will cancel the resize. 6. The Form can be resized and configured to display a different [layout](https://documentation.decisions.com/version-10/docs/layouts-overview). The default layout for the Form is the [Flex Layout](/version-10/docs/flex-layout) which adjusts component positioning and size based on content and screen size. 7. After the Form has been configured. Click **Save** to save changes to the Form and close the Form Designer.![](https://cdn.document360.io/6ef8bcc1-6489-4486-9ad1-83acff7e5df0/Images/Documentation/image-1779395128682.png) --- ## Feature Changes | Description | Version | Release | Developer Task | | --- | --- | --- | --- | | The Default Form has been updated with the following changes: - Adjusted the Main Container margins, row gap, and column gap. - Aligned the footer content to the right. - Updated the Secondary Button style to use an Outline button. | 10.1 | August 2026 | [DT-048075] |
